Billing Information

CBLPath bills Medicare, Medicaid and commercial insurance companies for procedures that we perform at the request of a patients' physician. Our invoice is separate from the physician's bill.

Each insurance carrier may send a patient an Explanation of Benefits (EOB). This is NOT a bill.

If you have any questions or concerns regarding a statement that you receive from CBLPath or an EOB from your insurance carrier regarding our service, please call the patient helpline at the number listed below.  We are available to assist you weekdays, 8:30 AM - 5 PM EST.

Please refer to website listed on your patient statement for payments.  We do not use any third party payment sites such as www.doxo.com as these could result in additional fees to our patients.
